Creatine monohydrate, 3 to 5 grams a day, alongside resistance training. That is the whole protocol for a woman over 30, and it is the same protocol as for a woman of 22. What changes after 30 is the reason to bother. Women carry roughly 70 to 80% less stored creatine than men, and muscle is the tissue you spend the next forty years trying to hold on to.
That is the case for it. Now the part the supplement aisle skips: the female-specific evidence is thinner and shorter than the marketing suggests. Creatine is one of the most studied compounds in sports nutrition, and most of that study was done on men.
Both things are true at once. Here is what the trials actually report.

Why creatine keeps coming up for women over 30
Creatine is made in your liver and kidneys, and the rest comes from meat and fish. Your muscles hold it as phosphocreatine, which is the fuel your body reaches for in the first few seconds of a hard effort. A heavy set. Standing up from a low chair when you are 70.
The 2021 review in Nutrients that mapped creatine across the female lifespan found women exhibit 70 to 80% lower endogenous creatine stores than men. Lower baseline stores are the reason researchers keep asking whether women have more room to gain from topping them up. It is a reasonable hypothesis. It has not been cleanly demonstrated.
The 30-something part is about timing. Muscle strength and muscle mass fall with age, which is what sarcopenia describes, and the interventions that slow it are the same at 32 as at 62. Starting earlier gives them longer to work.
What the trials in women actually found
The largest recent pooled analysis is a 2025 systematic review in Nutrients covering 69 studies and 1,937 participants. Across all adults, creatine plus resistance training beat placebo for bench or chest press strength (weighted mean difference 1.43 kg, p = 0.002) and squat strength (5.64 kg, p = 0.001). Then the authors split the data by sex. Leg press, squat, vertical jump and Wingate peak power all improved in men, and those same effects were not observed in females.
Read that carefully before you throw the tub out. "Not observed" in a subgroup usually means the female trials were smaller and fewer, so the analysis had less statistical power to detect an effect that may well be there. The honest reading is that the evidence in women is immature.
The trials designed specifically around women support that reading. A 2021 meta-analysis of 10 randomised controlled trials in 211 older women found creatine plus resistance training improved upper-body strength (p = 0.04), with no effect on lower-body strength or muscle mass overall. In the subset of trials that ran 24 weeks or longer, lower-body strength improved too (p = 0.03), alongside upper-body (p = 0.05).
In English: it works slowly, if you keep taking it and keep training. Six months, not six weeks.
The outcome that actually matters is the muscle you keep
Strength on a leg press is a proxy. The thing worth protecting across 40 years is lean tissue and the ability to use it.
A meta-analysis of 357 older adults across trials averaging about 13 weeks of resistance training found that adding creatine increased fat-free mass compared with training alone (p < 0.0001), along with chest press (p = 0.004) and leg press one-rep max (p = 0.02), and performance on the 30-second chair stand test (p = 0.03). That last one measures whether you can get out of a chair unaided.
The gap between a gym number and a life number is the whole argument for starting this at 35 instead of 65. Nobody loses the ability to stand up unaided in a single year. It goes in fractions, across decades, and each fraction is small enough to ignore until enough of them have stacked up. The things that slow it down are unglamorous and they compound: lift something heavy twice a week, and cover the nutritional inputs that training runs on.
The honest counterweight is the biggest and longest trial of the lot. Researchers put 237 postmenopausal women through two years of resistance training and walking with creatine or placebo. The result on the primary outcome, bone mineral density, was no effect at the femoral neck, total hip or lumbar spine. Some bone geometry measures held up better on creatine, and in the subanalysis of women who completed the protocol, lean tissue mass was higher than placebo (p = 0.046). A null result on the headline outcome and a modest one underneath it.
Direct imaging tells a similar story. A 2023 meta-analysis using MRI, CT and ultrasound to measure muscle thickness put the pooled effect of creatine plus training at 0.11, with a credible interval of -0.02 to 0.25. A very small effect, and one whose interval still touches zero.
Nobody selling creatine leads with that number. Worth thinking about.
What to take and what it will not do
Monohydrate. It is the form the research above used and the cheapest one on the shelf. Anything else costs more and carries less evidence.
Three to five grams daily, every day, including rest days. Loading is optional, and the classic dosing study is the reason we know that: 20 grams a day for six days raised muscle total creatine by about 20%, and 3 grams a day reached a similar 20% rise over 28 days. Loading gets you there in under a week. A steady 3 to 5 grams gets you to the same place inside a month. Timing across the day does not appear to matter much. Consistency across months does.
On safety, the International Society of Sports Nutrition position stand concluded that supplementation up to 30 g per day for five years is safe and well tolerated in healthy individuals. The 12-month bone trial in postmenopausal women reported no differences between groups in serum liver enzyme abnormalities, with normal creatinine clearance throughout. If you have existing kidney disease, that is a conversation for your doctor.
And it will do close to nothing on its own. Every trial cited here paired creatine with resistance training. It amplifies an existing stimulus. If there is no stimulus, there is nothing to amplify. The floor underneath the ceiling

Deficiency is the thing worth fixing first. A woman who is short of vitamin D and magnesium and taking creatine has bought the roof before the foundations.
So, is it worth it
Probably, if you are lifting. Three to five grams of monohydrate costs a few pence a day, the safety record across decades of trials is good, and the mechanism is well understood. The realistic return is a small strength gain and slightly better retained lean mass over months, most reliably past the 24-week mark.
Creatine is a small edge applied to work you are already doing. The work is the part that decides the outcome. Creatine is the rounding error on top, and after 30 the rounding errors are the whole game.
