Vitamin B12 deficiency symptoms: the nerve signs that come first

The first symptoms of vitamin B12 deficiency are usually neurological rather than blood related. Pins and needles in the hands and feet. Numbness. Balance that goes off in the dark. Memory lapses, low mood, and fatigue that a full night of sleep does not fix. Anaemia is the famous sign, and it tends to be the late one.

In 141 consecutive patients whose nervous system damage was caused by B12 deficiency, 40 of them, or 28%, had no anaemia and no enlarged red cells (Lindenbaum and colleagues, New England Journal of Medicine, 1988). Their blood counts read normal. Their nerves did not.

That is the reason this particular shortfall is worth understanding properly. Most nutrient gaps make you feel rough for a while and resolve once you fix the intake. This one can leave something behind.

The short version

Most people are not deficient

Start with the honest number, because most supplement content does not. Researchers analysed NHANES data on 12,612 US adults and found that roughly 1% had a clear vitamin B12 deficiency and about 92% had adequate status (Bailey and colleagues, American Journal of Clinical Nutrition, 2011). A further 5% had raised methylmalonic acid without low serum B12, a group that may include people with early deficiency.

The same paper makes a second point that matters more than the first. Depending on which cutoff you choose, the estimated prevalence of low B12 swings from 3% to 26%. That is how a supplement brand can honestly claim a quarter of adults are short, and how a sceptic can honestly claim almost nobody is. Same data. Different line in the sand.

In plain terms: the general panic is overstated, and the people who genuinely have a problem are a specific, identifiable minority. Worth knowing which group you are in.

Why the nerves go first

B12 has two jobs. It helps recycle homocysteine into methionine, and it is required to process methylmalonyl-CoA inside the mitochondria. When B12 runs short, methylmalonic acid and homocysteine both accumulate, which is why those two markers move before a blood count does.

The second job is the one that bites. B12 is needed to maintain myelin, the insulation around nerve fibres. Sustained deficiency damages the spinal cord in a pattern clinicians call subacute combined degeneration. That is where the tingling, the numbness, and the unsteady walking come from.

Lindenbaum's patients had markedly raised methylmalonic acid and homocysteine while their blood counts sat in the normal range, and 39 of the 39 who were followed improved once treated. Early is the operative word. Some B12 deficiency symptoms are severe, and some are irreversible (Pawlak and colleagues, European Journal of Clinical Nutrition, 2014).

Who is actually at risk

  1. Adults over 60. In large US and UK surveys, about 6% of people aged 60 and over were deficient, and closer to 20% had marginal status (Allen, American Journal of Clinical Nutrition, 2009). The cause is usually food-bound cobalamin malabsorption from gastric atrophy rather than low intake.
  2. People taking metformin. In the Diabetes Prevention Program Outcomes Study, low B12 at five years affected 4.3% of the metformin group against 2.3% on placebo, and low or borderline low B12 affected 19.1% against 9.5% (Aroda and colleagues, Journal of Clinical Endocrinology and Metabolism, 2016). Each year of use raised the odds by 13%.
  3. Long term users of acid blockers. A case control study covering 25,956 people with B12 deficiency found that two or more years of proton pump inhibitors carried an odds ratio of 1.65, and two or more years of H2 blockers 1.25 (Lam and colleagues, JAMA, 2013). That is an association rather than proof of cause, though stomach acid is needed to free B12 from food, so the mechanism is plausible.
  4. Vegans and vegetarians. A review of 40 studies found deficiency prevalence in adults ranging from 0 to 86.5%, with vegans consistently worse than other vegetarians (Pawlak and colleagues, 2014). Plant foods do not carry B12 in a usable amount. Supplementation is not optional on that diet.
  5. Anyone with pernicious anaemia or surgery affecting the terminal ileum. Antibodies against intrinsic factor stop B12 being absorbed, and no amount of dietary change gets around that (StatPearls, Vitamin B12 Deficiency).

The symptoms, roughly in the order they show up

The list below is drawn from the BMJ clinical review of vitamin B12 deficiency and the sources already cited.

  • Pins and needles or burning in the hands and feet, usually on both sides
  • Numbness, or a sense that your fingertips have gone slightly dead
  • Balance that deteriorates in the dark or with your eyes shut
  • Brain fog, trouble finding words, and short term memory slips
  • Low mood or irritability that does not track with anything in your life
  • A sore, smooth, red tongue
  • Fatigue and breathlessness on exertion, which is the anaemia arriving late

Notice the order. By the time the last item appears, the first has often been there for months.

What to do about it

Get tested before you supplement. Serum B12 is the standard first test and it is not a good one on its own, which is why methylmalonic acid is worth adding when your result lands in the grey zone between clearly low and clearly normal. Lindenbaum's series is the argument for that: serum B12 sat above 200 pg/mL in two of those patients despite established nerve damage.

Eat the foods. B12 comes from animal sources, and the NHS lists meat, fish, milk, cheese, eggs, and some fortified breakfast cereals, while putting the requirement for UK adults aged 19 to 64 at 1.5 micrograms a day.

If you are genuinely deficient, the dose is a different order of magnitude. In a randomised trial, 2mg of oral cyanocobalamin daily worked at least as well as 1mg injected monthly (Kuzminski and colleagues, Blood, 1998). A larger primary care trial in 283 patients aged 65 and over found oral dosing was no less effective than injections at eight weeks, with over 90% of both arms reaching normal levels (Sanz-Cuesta and colleagues, BMJ Open, 2020). That is treatment, and it belongs with your doctor rather than with a scoop of powder.

If you are not deficient and want to stay that way, the requirement is small and constant. Fireblood contains 2.5 micrograms of B12 as methylcobalamin, the form your body uses without having to convert it first. That covers the RDA. Being straight about the limit: one observational study of 299 healthy adults aged 18 to 50 found B12 biomarkers levelled off between 4.2 and 7.0 micrograms a day, and concluded the current RDA of 2.4 micrograms might be inadequate for optimal biomarker status (Bor and colleagues, American Journal of Clinical Nutrition, 2010). One study, an association rather than a trial, and it has not shifted the official figure. You should still know it exists before anyone tells you a daily multivitamin is the whole answer.

When to see a doctor

Persistent tingling, numbness, or a change in your balance is a reason to book an appointment rather than to order a supplement. Nerve symptoms are the ones with a clock running on them. This is general information rather than medical advice, and B12 is one of the few nutrients where that sentence is doing real work.

Your blood count is not a B12 test. Twenty eight percent of the patients in Lindenbaum's series would have been sent home reassured by one.
